1.                  2007 Summer Student Nurse Externship Program Skills Checklist (Click here, print and have your School of Nursing Complete).

2.                  Official registrar transcript through 2007 spring semester. 

3.         Official registrar copy of letter of good standing through 2007 spring semester.
